Records that document the paging of a physician for a patient. Information may include but not limited to patient name, date of birth, name of caller, physician requested and other related information. Format of records consist of paper and electronic.
Records of the "raw call data," which include, but are not limited to, the following data elements, originating phone number, destination phone number, length of call (rounded to next minute), originating city/state, destination city/state, date/time stamp.
Records that document the addition, modification, or removal of software from an computer system. File may include records that document operating systems; the in-house creation and modification of application programs; the structure and form of datasets; the use of commerical software packages; the structure of the system; system-to-system communication. File may include system overviews; operations logs; operator instruction manuals; system development logs; system specifications and changes (including narrative and flow chart descriptions); conversion notes; dataset logs; dataset inventories; dataset record layouts; hard copies of tables; data dictionaries; data directories; programming logs; program specifications and changes; records layouts; user views; control program table documentation; program listings; commercial software manuals; and other related records.
Records that document the maintenace of Emory's computer systems and is used to insure compliance with any warranties or service contracts, schedule regular maintenance and diagnose system or component problems, and document systems backups. File may include computer equipment inventories, hardware performance reports, component maintenance records (invoices, warranties, maintenance logs, correspondence, maintenance reports, and related records), system backup reports, and other related records.
Records that document the planning and development of Emory information systems. File may include information technology plans, feasibility studies, cost-benefit analyses, studies and surveys, system specifications and revisions, component proposals, technical literature, vendor literature and proposals, and other related records.
