Records that document the professional and regulatory issuance of credentials to individuals and facilities providing services within the student health center. File may include license applications, inspection reports, individual employee professional licenses, and other related records.
Retain until superseded then dispose of.
Confidential destruction is required. Paper must be shredded, pulped, or incinerated; electronic records must be overwritten or the storage media physically destroyed. Deletion of information in computer files or on electronic storage media is not sufficient.
