Daily Broadcast Logs

Records that document daily broadcast activities of the university radio station or television channel. Information may include: log sheets showing time signed on and off; any delays in broadcasting; engineer's name; announcer's name; and technical difficulties.

Data Input Records

Records that document data sheets used to create an electronic record. File may include service requests, such as work orders and mailing orders; surveys; instructor evaluations; tests; and other related records.

Deaccession Records

Records that document recommendations concerning deaccessioning of specific holdings and action upon those recommendations. Deeds

Records that document evidence of ownership of property. These records contain original documents that record names and addressees of grantor and grantee, description of property, date property was transferred to grantor, cross-referenced volume and page number of Superior Court Clerk's plat books, and signatures confirming transaction.

Demographic Reports

This series documents reporting of enrollment, demographic, and geographic information about students who are attending or have attended Emory. This series may include, but is not limited to: original data sets, submission syntax, and reports.

Deposit Statements

Records that document financial transactions between a bank and an unit. File may include correspondence, bank statement and other related records.

Depreciation Records

Records that document the useful life and depreciation of Emory owned equipment and property, usually for insurance purposes.

Development and Endowment Management Fee Records

Records that document the revenue earned quarterly on gift accounts and fees charged by the university's development office. Information may includes dates, gift account amounts and identification numbers, earnings, account responsible for payment, fee amount, and other related records.

Development Records

Records that document Emory operations relating to fundraising, donor identification and solicitation, corporate campaigns, gift planning and external promotions. Records that document the listing of current employees, students, or faculty, and or phone offices, and service providers listings.

Donor and Development Records

Records that document the efforts of Development Office to establish relationships with alumni, the community groups, individuals, and businesses, to gain their assistance with the development and coordination of institutional programs. Records that document the terms and conditions for loans of manuscripts or artifacts to or from collections, including records documenting chain of custody. File may include correspondence, loan agreement and other related records.

Drug Inventory: Controlled Substances

This series documents the order, procurement, use, inventory, and disposition of controlled substances for use in research and teaching. This series may include, but is not limited to: order logs, receipts, use logs, and disposition instructions.

Employment Eligibility Verification

Records that document information used to establish the identity and to verify the employment eligibility of employees to preclude the unlawful hiring of persons not authorized to work in the United States. Records include completed I-9 records and copies of documents that establish the identity and the employment eligibility of the employee.

Endowed Account Reports

Records that document the responses to inquiries for evidence of employment from offices within the university, banks, and credit agencies, government agencies, businesses, and current and former personnel. Records may include copies of written requests; release of information authorization records; telephone request records, office control documents; statistical summaries; correspondence; and other related records.

Endowed Scholarship Records

Records that document endowed scholarship to eligible students. File may include scholarship requirements, recipient application, financial information and other related records.

Endowment and Trust Fund Agreements

Records that document how monies are to be used in accordance to the endowment or trust fund. File may include the agreements, correspondence, and other related records.

Environmental Health and Safety Training Records

This series documents employee training and certification such as for equipment operation, hazardous material handling and emergency procedures, driver training, CPR and first aid training, and asbestos awareness training for removal, abatement, or transportation. This series may include, but is not limited to: sign off sheets indicating that employees have received training; and instruction sheets.

Environmental Regulation Reports

This series documents compliance with environmental laws and guidelines set by federal, state, or local government such as the regional air pollution authority, Environmental Protection Agency (EPA), and Georgia Environmental Protection Division. This series may include, but is not limited to: Title V air discharge permits and air quality reports; waste water discharge applications and permits; water quality reports; and notices of violation and non-compliance.

Equipment Inventory Records

Records that document the acquisition, location, transfer, and disposition of Emory-owned property and equipment. File may include equipment inventory lists, lost/stolen property reports, damage or loss property claim, equipment transfer records, correspondence, and other related records. 

Equipment Loan Agreements Records

Records that document the lending of equipment to borrowers conducting research, educational programs, and or other activities consistent with Emory goals and missions. File may include signed loan agreements, correspondence, and other related records.

Records that documents the operation, maintenance, service and repair of university equipment. This series documents equipment maintenance and functioning. This series may include, but is not limited to: calibration and validation, maintenance and inspection, operating instructions and manuals.

Exhibit Records

Records that document the display and use of artifacts and materials held by the collection units or displays created by the units. File may include research materials concerning the cultural environmental setting surrounding the artifacts, bibliographies, lists of artifacts or items considered for inclusion, drafts of exhibit descriptions or scripts, publicity or advertisements for exhibits, artifact labels, photographs of exhibits, exhibit renderings and layout diagrams, exhibit scheduling and transport information, contracts and agreements, evaluation records, condition records, exhibit assembly and presentation instructions, correspondence, and other related records.

Expenditure Ledger and Supporting Documentation

Records the document expenditures and purchases for an unit. File may include photocopies of purchase requisitions, maintenance contract, paid invoices, freight bills, order forms, packing lists, travel expense, and other related records.

Records that document a faculty member accomplishments for a year. Report may contain courses taught, articles written, extra curricular activities, projects, and other related information. Report should be filed in faculty personnel file.

Faculty and Student Handbooks

Records that document the requirements, policies and offerings of specific instructional units for use by current or potential faculty or students. File may include policies on: fields of study, faculty, academic requirements, the evaluation process, and the research process

Faculty Annual Review

Records that document a faculty member's work performance. File may include correspondence, letters of commendation, written reports, and other related records. 

Faculty Council Records

Records that document the proceedings and actions of faculty council. File may include minutes; studies; recommendations; resolutions and enactments; reports; correspondence; and other related records.

Faculty Exchange and Visiting Scholars Records

Records that document a program which allows one or more faculty exchange and or visiting scholars to assume residence on campus for an academic year or a shorter duration. File may include advertisements, applicant data, arrangements and schedules, publicity and news clippings, presentation audio recordings, correspondence, and other related records.

Faculty Staff Assistance Program Records

This series documents professional counseling and consultative services provided to faculty and staff. This series may include, but is not limited to: presenting complaint or basis for request for services, documentation of informed consent for treatment, assessment, psychological testing, structured interviews notes, and treatment or work plan.

Family Educational Rights and Privacy Act (FERPA) Records

This series documents student requests and consent authorizations or denials in accordance with the Family Educational Rights and Privacy Act (FERPA). Records that track the monthly income and expenses of a department.

FAS Revision Reports and Supporting Documentation

Federal Title IV Program Records and Institutional Records

Records that document eligibility to participate and school's administration of Federal Title IV and programs. Records may include Program Participation Agreement; recertification; education program eligibility; accreditation reviews and reports; audits and program reviews; other records, as specified in regulation, that pertain to factors of financial responsibility and standards of administrative capability; and consortia agreements between and among schools; and other related records.

Fee Assessment Records

Records that document the payment of fees for course fees, lab time, diplomas, commencement exercises, and other related records.

Fellowship Award Records

This series documents the administration of student fellowships. This series may include, but is not limited to: agreements/contracts with departments, postings/notices, applications, and student resumes.

Film, Videotapes, and Sound Records

Records that document visual and/or aural documentation of Emory activities and events including intercollegiate athletics, students, faculty, and staff with significant relevance to either to Emory or individual unit's function and mission. These productions may have been created for recruitment and orientation, fund-raising, publicity, research, or teaching. File may include identified and unidentified videotapes; motion picture films; phonographic recordings (16, 33, 45, and 78 RPM formats); audio tapes (all formats); compact audio discs; and transcriptions when available. This series documents preparation for and response to fire events on campus. This series may include, but is not limited to: fire drill preparation material, safety inspection reports, and evacuation plans.

Fire Protection System Contract Records

This series documents the selection, purchase, and installation of fire alarms, sprinklers, fire pumps, extinguishers, and commercial hoods. This series may include, but is not limited to: requirements, bid records, plan reviews, project schedules, contract changes, consultant contracts, and budgets.

First Destination Surveys

This series documents the surveying of recent graduates. This series may include, but is not limited to: questionnaires that provide the following information on individual alumni: career status or job title, continuing education, geographic location, source of finding employment, relationship of employment to major, and salary.

Fiscal Records and Reports

Records that document Federal Title IV Aid transactions, including receipt, management and disbursement of funds. Records may include all Title IV program transactions; bank statements for all accounts continuing Title IV payments, cash disbursements, refunds, and repayments; general ledger (must be separate from school's other financial transactions) and related ledgers that identify each Title IV program transaction; Federal work-study payroll records; Annual Federal Fiscal Operations and Applications for Funds Report (FISAP). Food and Alcohol Inventory Records

Records that document the practice, playing, and attendance statistics about each game and the season for each sport by playing year. Garnishment Case Files
