Data Input Records
Records that document data sheets used to create an electronic record. File may include service requests, such as work orders and mailing orders; surveys; instructor evaluations; tests; and other related records.
Retain until information is inputted and quality control has verified correct entries then dispose of.
Confidential destruction is required. Paper must be shredded, pulped, or incinerated; electronic records must be overwritten or the storage media physically destroyed. Deletion of information in computer files or on electronic storage media is not sufficient.
