A report should reside in the patient medical file. The pathology department should retain cytology reports for ten years after report was written then dispose of. Records of number of slides examined by an individual in a 24-hour period and number of hours each person spends examining slides in 24-hour period should be maintained 5 years.

Confidential destruction is required. Paper must be shredded, pulped, or incinerated; electronic records must be overwritten or the storage media physically destroyed. Deletion of information in computer files or on electronic storage media is not sufficient.
